How to join the Corporate Cards Programme?

To join the Corporate Cards Programme, you should:

1. complete the Corporate Card Programme Agreement application form in 2 copies (download the form)
2. complete the application form for the product you want to use (2 copies)
3. send the Agreement and the application(s) to Diners Club Polska together with the following documents:
   • copy of the trade register entry certificate
   • NIP (tax identification no.)
   • last year’s balance sheet and income statement (P&L account)
   • relevant last year’s PIT return (if you are self-employed)

You should send these documents as true copies certified for conformity by an authorised company employee.

• statement on no arrears in payments from ZUS (Social Security Institution) and Inland Revenue (or the STATEMENT OF INCOME form)
• declaration of timely settlement of dues (Diners Club form)
