What documents do I have to submit in order to obtain a Classic Diners Club Private Card?

Applicants employed upon a job contract*: 2 copies of a legibly completed application form copy of your ID card confirmation of the employment status and a statement of the average remuneration in the last 3 months (DC form).

Self-employed applicants*: 2 copies of a legibly completed application form copy of your ID card copy of the trade register entry certificate statement on profits in the current financial year (DC form)

* in specific situations, Diners Club Polska reserves the right to require additional documents to confirm the Applicant’s creditworthiness.
